What is menopause delay and reproductive longevity?

Menopause occurs when the ovaries gradually lose their remaining follicles and stop producing the hormones that regulate fertility and the menstrual cycle. Although menopause typically occurs between the ages of 45 and 55, ovarian aging begins years earlier as the ovarian reserve and egg quality show accelerated decline after women reach their thirties.

Reproductive longevity is an emerging area of medicine focused on preserving ovarian function for as long as safely possible. Our researchers are studying ways to maintain natural hormone production, extend reproductive potential, and support healthy aging. While many of these approaches are still investigational, advances in reproductive medicine continue to expand what may be possible. This growing field reflects a shift toward helping women maintain ovarian health throughout different stages of life, rather than focusing solely on fertility treatment.

Why are researchers studying menopause delay?

Preserving ovarian function may one day offer benefits beyond fertility. Potential advantages being studied include:

  • Extending reproductive potential
  • Maintaining natural hormone production
  • Reducing symptoms associated with menopause
  • Supporting healthy aging and quality of life
  • Extending overall lifespan

Research continues to explore how ovarian aging influences overall health and whether preserving ovarian function may provide long-term benefits for women's reproductive and general health.

What approaches are being explored?

At Innovation Fertility and his research lab at Yale School of Medicine, Dr. Kutluk Oktay and his team are investigating several strategies to support reproductive longevity, including:

  • Ovarian tissue cryopreservation, which preserves ovarian tissue while a woman is younger for potential future use.
  • Ovarian tissue transplantation, an established treatment for selected patients that can restore ovarian function and fertility using their previously frozen tissue.
  • Ovarian longevity therapeutics, which are investigational medications designed to help preserve the ovarian reserve.
  • Emerging technologies, such as in vitro growth of primordial follicles (ivg-p)  that may further expand fertility preservation options in the future.

Is menopause delay currently available?

While ovarian tissue cryopreservation and transplantation are established fertility preservation options for medical indications, treatments designed specifically to delay menopause remain investigational. We continue to study their effectiveness and long-term outcomes before they can become part of routine clinical care.
